A custom modeline isn't needed; you just need to add 1280x768 to the available resolutions in the Screen section. Changing the default 1280x1024 entry to 1280x768 should be sufficient.
The Monitor section on the frontend with this monitor still has the settings I put in there for a Samsung 215TW; any combination of HorizSync and VertRefresh that allows 1280x768 at 60 Hz should work. I'm using 30-81 (kHz) for HorizSync and 56-75 (Hz) for VertRefresh.
